Looking to Renew your South Dakota Licensure?
 
The South Dakota Board of Osteopathic & Medical Examiners Website for Renewal can be found at http://medicine.sd.gov.

Are you new to South Dakota? Did you just graduate? Here is information on initial licensing requirements:

Select the type of license and follow those instructions completely. The application must be completed in its entirety and submitted with the appropriate fee or it will be returned to you. Ignore the "Certification" section, it is no longer applicable and will be removed when this form is revised.

The applicant is responsible to request verifications from each state board, the college of graduation and the most recent past employer.

  • The Board must receive verifications from every state board: for all licenses, permits, certificates; whether active or inactive. These must be requested by the applicant but sent from the state board directly to our office, there is usually a fee to do this process.
  • The Board must receive directly from your college of graduation, the completed form, "Verification of Graduation". Do not forget to attach a picture.
  • The Board must also receive from your most recent past employer, the completed form, "Verification of Employment" prior to licensure. 

Please be aware that you may not engage in any type of practice, even under supervision, until you obtain your license and receive notification from this office.

**UNLICENSED PRACTICE AS A DIETITIAN/NUTRITIONIST IS A CRIMINAL OFFENSE IN VIOLATION OF SDCL 36-10B.   THE BOARD INTENDS THIS NOTIFICATION TO SERVE AS NOTICE UPON YOU THAT UNLICENSED PRACTICE IS UNPROFESSIONAL CONDUCT, A VIOLATION OF THE ABOVE CODIFIED LAWS, AND GROUNDS FOR DENIAL OF LICENSURE.**

Dietetics/Nutritionist License

I. Full License (see below II. Temporary Permit only for those waiting to take the Examination.)

Dietetics

  1. Complete application form and file with Board office.  Ignore the "Certification" section, it is no longer applicable and will be removed when this form is revised.
  2. Submit $70 fee.
  3. Submit Verification of Graduation from dietetics program (complete online and print for mailing).
  4. Request verification from the CDR of current registration with the Commission on Dietetic Registration to be send directly to the Board office.
  5. Submit verification of at least 900 hours of U.S. pre-professional practice under the supervision of registered dietitian or individual who holds a U.S. or equivalent doctoral degree in human nutrition, dietetics, nutrition education, food and nutrition, or food system management.
  6. Submit Verification of Employment from most recent past employer.
  7. Request Verification(s) of Licensure to be sent directly from state board to Board office, there may be a fee involved.
  8. Request to the CDR that examination scores are sent directly to the Board office.

Nutrition

  1. Complete application form and file with Board office. Ignore the "Certification" section, it is no longer applicable and will be removed when this form is revised.
  2. Submit $70 fee.
  3. Request Verification of Graduation from masters or doctoral nutrition program to be sent directly to Board. Attach picture.
  4. Submit Verification of Employment from your most recent past employer.
  5. Request Verification(s) of Licensure to be sent directly from state board to Board office, there may be a fee involved.
  6. Request to the CDR that examination scores are sent directly to the Board office.

II. Temporary Permit only for those waiting to take the Examination. Submit all of above requirements for full licensure except examination scores and registration with the Commission on Dietetic Registration.

    1. Complete application form and file with Board office. Ignore the "Certification" section, it is no longer applicable and will be removed when this form is revised.
    2. Submit $50 fee.
    3. Submit Verification of Employment from temporary employer.

Prior to issuance of full licensure, the Board must have:

    1. Examination scores. Request to the CDR that examination scores are sent directly to the Board office.
    2. Registration with the Commission on Dietetic Registration. Request verification from the CDR of current registration to be send directly to the Board office.
